Children’s Art Tax Credit (CATC)

The Music Room will be issuing receipts for music lessons paid in 2011.  To ensure you receive your reciept in a timely manner please complete the form below.  You must complete one form for each child.

Required information for CATC receipts:

2. Child Last Name
3. Child Birth Date (dd/mm/yyyy)
4. Payer First Name
5. Payer Last Name
6. e-mail address
